Submit an Excuse for a Student Absence
Parents/guardians may submit excuses for up to five (5) days of absences per year without a doctor's note.
After five (5) cumulative parent excused absences, a student must present documentation from a medical provider of personal illness for any further absences to be excused due to illness.
Please provide excuses for your student's absence(s) and upload any necessary documentation below. Please complete a separate form for each student.

Submit a Transportation Change
***ALL transportation changes must be submitted by 2:30pm. If you need to submit a transportation change after 2:30pm, please call the ICMS front office at 770-385-6453.***
